Release of Medical Information
Patients’ medical records are kept strictly confidential. Before we can release any records, patients must sign an authorization form. If you are scheduled for a diagnostic test, please request your X-rays with at least a 24-hour notice.
Appointments
Our patients’ time is valuable. Appointments and surgeries are scheduled promptly. Patients are seen by appointment only, except in cases of an emergency. Please call as far in advance as possible to make an appointment. If you cannot keep an appointment, please notify the office at least 24 hours in advance of the appointment time.
Registration
New patients are required to register before their first appointment by calling our receptionist. Be prepared to provide insurance and demographic information during the registration process. If you are a returning patient, please notify the receptionist of any change in name, address, telephone numbers, or insurance coverage.
